Situs inversus totalis (SIT) is a rare condition characterized by the mirror image location of all of the thoracic and abdominal organs. There are only a handful of reports documenting the presence and repair of an abdominal aortic aneurysm in the setting of SIT. Here, we present a rare case of a juxtarenal abdominal aortic aneurysm repaired through a retroperitoneal approach in a patient with A juxtarenal abdominal aortic aneurysm occurs in the abdominal aorta and extends up to, and includes, the lower part of the kidney arteries. These generally require open repair but can be repaired with a custom endovascular graft. Lakeview Regional Medical Center A juxtarenal aneursym occurs near the renal (kidney) artery. A 79-year-old male who had infrarenal endovascular repair of abdominal aortic aneurysm (EVAR) 13 years previously presented with leaking juxtarenal AAA. Emergency fenestrated EVAR (FEVAR) was performed utilizing a stent graft designed and built for a different patient. Juxtarenal: A Juxtarenal aneurysm is one that starts where the arteries to the kidneys come off the aorta as opposed to 95% of such aneurysms which have a "neck" or relatively normal segment of aorta below the renals. This makes open or endovascular repair more difficult. Before elective repair, clinical consideration of coronary artery disease (CAD) is often needed and may or may not require further evaluation (see table Tests for Assessing Cardiac Anatomy and Function) because some patients with an abdominal aortic aneurysm have significant risk of … 2015-10-28 A juxtarenal abdominal aortic aneurysm occurs in the abdominal aorta and extends up to, and includes, the lower part of the kidney arteries.
Objective: Fenestrated endovascular aneurysm repair (FEVAR) is a well established treatment for complex abdominal aortic aneurysms (AAAs). FEVAR with custom made devices (CMDs) has limitations in both the emergency and elective settings due to time consuming manufacture. “Off the shelf” (OTS) fenestrated stent grafts are a potential solution.

Juxtarenal aortic aneurysm (JRAA) is an unusual, but not rare, pattern of aneurysmal disease of the abdominal aorta in which dilatation extends up to, but does not involve, the renal arteries. The objective of this report was to retrospectively analyze experience with JRAA repair at a tertiary referral center over a 5-year period. Repair of juxtarenal aortic aneurysms, if compared with infrarenal aortic aneurysms, is characterized by more extensive mobilization of viscera to achieve adequate exposure of the abdominal aorta and by a period of renal ischemia, potentially increasing operative risk, especially the risk of postoperative renal dysfunction. The current standard for endovascular juxtarenal aortic aneurysm repair employs custom fenestrated endografts with fenestrations (holes) or scallops (U-shaped opening in the top of the graft) that are aligned with the ostia of the renal vessels, and often the SMA, with or without bare or covered stents placed into these vessels. Studies that reported outcomes of more than 10 cases of juxtarenal aortic aneurysm following f-EVR or open surgical repair were eligible for inclusion.

Studies that reported outcomes of more than 10 cases of juxtarenal aortic aneurysm following f-EVR or open surgical repair were eligible for inclusion. Outcomes of interest were 30-day mortality, renal impairment, target vessel patency, length of stay and secondary re-intervention rate. Studies of ruptured abdominal aortic aneurysm were excluded.
Endovascular abdominal aortic aneurysm repair (EVR) is associated with low peri-operative morbidity and mortality in patients with suitable aneurysm morphology. At least 55% of patients have aneurysms amenable to conventional EVR. 1 An inadequate landing zone for the graft below the renal vessels precludes conventional EVR requiring the patient Repair of a juxtarenal abdominal aortic aneurysm in a patient with situs inversus totalis using a retroperitoneal approach Matt A.
